Prediction and prevention of complications after liver resection

• Two principles of immunomodulatory effect of the red laser irradiation of intravascular blood were discovered: irradiation effect depends on primary status of immune system (decreases  stimulated or stimulates immunosuppressed system); and irradiation effect peaks on 3-4 days and disappears after 7 days.
• Red laser irradiation of intravascular blood corrects coagulation changes and increases liver regeneration.
• Water soluble based ointments and red laser irradiation of intravascular blood are useful for early management  of frostbite
• In cirrhotic liver resections combined application of CUSA (Cavitron Ultrasonic Surgical Aspirator) and argon beam coagulator is more useful than alone.
• Parameters (diameter and pressure) of water-jet for liver resection  were studied
• Regeneration dynamics of normal and cirrhotic livers after resection were studied 
• The regeneration stimulators have little effect on normal liver regeneration, but it is needed for cirrhotic and hepatitis livers and blood laser irradiation may be useful.
• Intraoperative indocyanine green test is useful for prediction of posthepatectomy liver failure.
• Experimental models for liver cysts and abscesses were developed.
• Most of postoperative complications in living liver donors are of mild degree, and biliary complications are more frequent.
• A retrograde trans-anastomotic external drainage of recipient side of choledochus is more useful for decrease of bile leakage in liver recipients.
• Laparoscopic approach decrease complications and hospital stay in both complicated and uncomplicated appendectomies.
• Dissection of Calot’s triangle (two windows technique) with artery ligation first significantly decreases complications and may be referred as standard technique for laparoscopic cholecystectomy.
• In patients with cholecysto-choledocholithiasis  one-stage laparoscopic common bile duct exploration and cholecystectomy has surgical and economical advantage over two-stage approach.
• Simultaneous laparoscopic operations decrease common postoperative complication rate and do not increase specific complications.
• Laparoscopic fundoplication is first choice for treatment of Barret`s esophagus.
• Laparoscopic approach is first choice for treatment of hydatid cysts irrespective number, complication and location.
• Laparoscopic hernioplasty significantly decreases wound complication rate and may refer as first choice for treatment of postoperative abdominal hernias.
• Intrarterial infusion of autologous mesenchymal stem cells is useful for treatment of liver cirrhosis.
• Splenic stiffness by elastography is diagnostic value for portal hypertension.
